📋 Tanggung Jawab Pekerjaan

• Develop and implement comprehensive case management plans for patients with mental health conditions.
• Coordinate and manage patient care across multiple healthcare providers, ensuring continuity of care.
• Monitor patient progress and adjust care plans as needed to achieve optimal outcomes.
• Advocate for patient needs and ensure their rights and preferences are respected.
• Collaborate with healthcare professionals to identify and address patient barriers to care.

🎓 Kualifikasi & Syarat

• Bachelor's degree in Social Work, Psychology, Nursing, or a related field.
• Minimum of 3 years of experience in case management or a related field.
• Certification in Case Management (e.g., CCM, CPC) is a plus.
• Strong knowledge of mental health care principles and best practices.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with healthcare professionals, patients, and community stakeholders.
• Proficiency in using electronic health records (EHR) systems like Epic and Cerner.
• Fluency in Bahasa Indonesia and English is required.
